Online Form - Request A Copy Of My Rates Notice

Use the below form to request a copy of your rates notice or a rating statement per assessment (listing relevant rating years and amounts). Please note that reprints of rates notices are free for the current rate year however there is a charge of $24 per year for previous years' rates notice reprints and a $24 fee for a rating statement. If requesting either of these, please contact Administration on 9576 4600 to pay the fee after making your request.

A copy of your rates notice or rating statement can be either posted or emailed to you as per your preference on the form below.

Please note that we can only provide rating information as far back as 1998/1999.
